What Is a Social Security TPQY Form?

A Social Security TPQY form is a form used by a third party to verify Social Security benefits received by an individual in determining qualification for other benefits. The process of receiving the information requested is done online.
An entity may perform a TPQY request on an individual when the entity is looking to verify that person receives some type of benefit from the Social Security Administration. For example, in the state of Connecticut, a TPQY request may be performed on a person to verify he is disabled in order to receive certain property tax exemptions available for blind or disabled individuals.
