Doctors write initial notes during a patient’s first visit. On the succeeding visits, the doctors write progress notes to record any changes or developments.
Dr. Shmerling advises that it is better for patients to ask their doctors directly via phone or email about any concerns they might have than to try to read and interpret medical records on their own.
