Why Is the Member 4 Copy of the DD Form 214 Important?

The Member 4 page of a DD 214 form is often the most important to separated service members because it is almost always requested as part of an application for veteran benefits. Employers may also request to see it as it contains information about the service member’s discharge.

The Member 4 portion of the DD 214 lists the type of separation, the character of service at discharge (honorable or otherwise) and the reason for separation.

Also listed on the page are any military education received, their primary duties, their medals and campaign ribbons, and their dates of service and separation.
