In addition to SRC, RNA Pol III antibody also has a temporal relationship to cancers (plus or minus one year). Some (e.g., some researchers at Johns Hopkins) believe that RNA pol III scleroderma is a paraneoplastic syndrome (a response to cancer, which may or may not be diagnosed).

Over the past 35 years, however, several additional antibodies have been isolated that are related to the scleroderma family of diseases. Some of these more recently isolated antibodies are specific to scleroderma, for example, anti-RNA polymerase III and anti-Th/To.

In particular, anti-RNA polymerase III (anti-RNAP III) has been associated with diffuse cutaneous disease, scleroderma renal crisis, a temporal relationship with malignancy, myositis, synovitis, joint contractures, and gastric antral vascular ectasia. However, anti-RNAP III has not been associated with systemic sclerosis sine scleroderma.

To the Editor: The study by Cordullo, et al investigated the prevalence of systemic sclerosis (SSc)-related autoantibodies among SSc patients with scleroderma renal crisis (SRC) in Italy. The authors report that the majority of the SRC patients had anti-topoisomerase I (anti-topo I) antibody (30/46, 65%), whereas a minority of patients (7/46, 15%) had anti-RNA polymerase III (RNAP) 1.

Results. Among 2,177 patients with SSc, 7.1% had a history of cancer, 26% were positive for anticentromere antibodies (ACAs), 18.2% were positive for anti-Scl-70 antibodies and 26.6% were positive for anti-RNA polymerase III (anti-RNAP) antibody.

Hi Ruth, I also tested positive for RNA Polymerase about a year and a half ago and was diagnosed with SSc. I had read a bit about scleroderma and the different autoantibodies and was quite scared to come back positive on this one, so I know how you must be feeling.

Scleroderma patients with RNA polymerase III (RNApol3) antibodies are at increased risk of cancer within three years of diagnosis, most notably for breast and lung cancers. New scleroderma-specific antibodies such as eIF2B, RuvBL1/2, and anti-BICD2 are infrequent, but are associated with unique clinical phenotypes.
