Ive had several pts with multiple different diagnoses and admitting problems whose INR continued to increase daily despite holding Lovenox, Coumadin, etc. One pt had daily INRs of 6.3, then 7.1, then 7.4. All the while holding any meds that would increase it. Docs wanted to wait and see if it wou...
Warfarin. Warfarin is taken by mouth to inhibit vitamin K. This vitamin is essential for effective production of clotting factors II, VII, IX, X, and anticoagulant proteins C&S. Warfarin is given once daily. It is monitored by the prothrombin time and the international normalized ratio (INR). Warfarin is a narrow therapeutic index drug (NTI).
dose without INR monitoring during the first few days of therapy. Raise the INR gradually The INR response after starting warfarin is variable and highly dependent on the half-lives of the vitamin K-dependent clotting fac-tors. The initial increase in the INR (the anti-coagulant effect) is primarily a response to
Increase or decrease oral Vitamin K administration based on the clinical situation. Parenteral Vitamin K should only be used for life-threatening bleeding and is therefore not appropriate for outpatient use.
Alcohol intake greater than 3 drinks daily can increase the effect of Coumadin. As long as alcohol intake does not exceed 3 drinks daily, clotting times should not be affected. This amount of alcohol is present in 12 ounces of table wine or three 12 ounce beers.
Foods that increase or decrease inr levels. ... For example, it's common for INR to increase based on the same warfarin dose as winter approaches due to the increase in root and other vegetables not containing vit K. Similarly the INR will decrease in the spring and summer as more leaf veg like salads are consumed. ... Would there not be foods ...
and 3.1 on 2.5 mg of Coumadin. On day 4, and her INR was 3.3, her Coumadin was held. Repeat INR was 3.3 on day 5 without Coumadin. INR was 1.8 on day 6 and she was given 2 mg of Coumadin. Her INR suddenly increased to 6.9 and 7.2 on day 7 and 8 respectively without Coumadin (Figure 1). On day 6, she complained of right facial pain.
INR (international normalized ratio) tests are important in helping your healthcare provider determine your response to COUMADIN. INR tests check to see how fast your blood clots. The INR test is primarily used to monitor warfarin therapy, where the aim is to maintain an INR in target range.
Potentiation of anticoagulation • Monitor INR closely with the addition and withdrawal of lactulose. Phenytoin Decreases warfarin protein binding, leading to increased free warfarin • Monitor INR closely during the two to three week interval following the addition or deletion of phenytoin therapy. (Initial increase in warfarin effect, then
Your, right. Typicallly we see food that can decrease INR, not as many things increase INR. I am currently working and jsut asked a couple of our coumadin pharmacist. They have seen hot pepers like jalapenos, sunflour seeds, grapes, ice tea, real licorice, and theoretically craneberry juice. I will do some futher reasearch to provide a list.
