He called these the "Vestiges" basically, magic items created long ago that scaled in power. Basically there is a standard 'dormant' magic item stage, and depending on certain conditions, they would reach into an "Awakened" stage, followed by the "Exalted" stage unlocking additional abilities, or damage, or higher AC for example.

Yes. Exalted does indeed stack. If you have more than 1 instance of Exalted, then the creature that attacks alone will gain +1/+1 for each instance of Exalted until the end of turn. So, if you have 4 instances, the single attacking creature would gain +4/+4 until the end of turn. Exalted is a very powerful ability.
