Medical documentation and a medical insurance statement must be included with the form. Other documentation to include is a copy of the invoice for the NHTSA installer's work, proof of customer payment for the invoice, and the vehicle purchase or sale agreement. A reimbursement of up to $1,000 is possible from Nissan, providing all documents are received along with the required form.

Only one reimbursement per vehicle will be made. The program applies only to vehicles sold in the United States. The adaptive equipment must be medically necessary to operate a Nissan vehicle or to transport passengers with a physical disability.

The Nissan Mobility Program does not reimburse for instillation if the adaptive equipment is covered by medical insurance. The NHTSA provides a list of registered installers and information about adaptable equipment. This same information is available online through the Nissan Mobility Assist website.
