The MCS-150 form is a motor carrier identification report. The Federal Motor Carrier Safety Administration requires companies wishing to establish a certain type of business to fill out this form before doing so.
The MCS-150 form must be filled out by any business that operates as a shipper, freight forwarder, motor carrier, cargo tank facility or broker. The 15-page form can be printed out and filled in or done online at the FMCSA site. Some companies may also have fill out form OP-1 or OP-2, which the FMCSA also administers. The FMCSA enforces regulations such as the Federal Motor Carrier Safety Regulations and the Hazardous Materials Regulations.
