Once you click the blue link, the page redirects to a web form. This form asks for a first name, last name, user name, password, location, birthday, gender and phone number. You can use an existing email address as your Outlook user name. After this information is submitted, type in the CAPTCHA found at the bottom of the form. Click the blue bar that has the Create Account link in white text.
