HCR ManorCare uses the PointClickCare platform to meet the needs of individuals discharged from the hospital who require further nursing and/or rehabilitation care, and the needs of the professionals who care for these patients. The online platform provides an electronic health record, medication management, nutritional management and practitioner engagement. It also provides "point-of-care" services, including access to key information, task lists and schedules, and complete documentation.
HCR ManorCare, with the aid of the PointClickCare technology, connects the providers that contribute to care management and delivery, enabling a collaborative approach to heath care in a senior living community. The cloud-based platform is used for both long-term and post-acute care facilities, as well as primary care offices, therapy providers, labs, hospitals, pharmacies and health information exchanges.
HCR ManorCare encompasses over 500 skilled nursing and rehabilitation centers, memory care facilities, assisted living facilities, outpatient rehabilitation clinics and hospice/home health care agencies. Most of the organization's facilities operate under the ManorCare Health Services, Heartland and Arden Courts names. The group of facilities and practices includes over 55,000 employees, nearly 300 Medicare and Medicaid-certified rehabilitation and nursing centers, and hospice care in more than 100 markets. ManorCare was founded in 1982 in Indiana and acquired by HCR in 1998.
