Numerous free online design questionnaire tools exist, two of the most popular and accessible being Google Forms and Survey Monkey, as of 2015. Google Forms is free to use for anyone who has a Google account. Users can access it visiting Drive.Google.com, clicking New, and selecting Google Forms. Survey Monkey has a free version for questionnaires with up to 10 questions, 100 respondents, and 15 common question types.
Google Forms includes branching and skip logic. This feature allows users to ask respondents different questions depending on their answers to earlier questions. The responses to Google Forms collect in a Google spreadsheet. The most significant drawback to Google Forms is that analysis of the results is entirely up to the user; however, for anyone who is reasonably comfortable with spreadsheets, this may not be an issue.
Survey Monkey's free version is far more limited than Google Forms; however, it provides a reasonable suite of analysis tools. One drawback is that there is no way to export data on the free version.
Some other free tools include Typeform, which is excellent for highly creative questionnaires, and Zoho Survey, which is quite similar to Survey Monkey but offers more in its free version.
