Asurion requires customers to submit information to confirm their identity and support the validity of their claim request. Customers must provide Asurion with the make and model of their device and the last four digits of their Social Security number. The insurance provider may also ask customers to present additional information, such as an affidavit, proof of ownership or personal ID, before it can complete the claim.
