Download the Boy Scouts of America Annual Health and Medical Record form by visiting the organization's website at Scouting.org and navigating to the Scouting Safely section of the webpage. Locate the Forms drop-down menu on the left side of the page, and scroll down until you see the hyperlink for the Annual Health and Medical Record. Click through the link, then download either the All Scouting Events form or the Are You Going to Camp form, as applicable.
The All Scouting Events medical form is required for all Boy Scouts who are involved in the essential Scouting activities, including local tours and weekend camping trips that are fewer than 72 hours in duration. The Are You Going to Camp medical form, on the other hand, is intended for use for Scouts who attend one of the various Boy Scout summer camps or for Scouts who are involved in camping trips that exceed the 72-hour threshold. This medical release form is slightly more detailed and involves a pre-participation physical assessment.
The Annual Health and Medical Record website provides a number of additional forms that are intended for specific High Adventure Trips such as scuba trips at the Florida Sea Base, canoe and other extended treks with Northern Tier, and participation in the Philmont Summer Camp.
