If you choose to take the lock to a Master Lock retailer, make sure that it is not attached to anything. Have the dealer call 1-800-308-9244 on your behalf. While Master Lock does not charge for this service, the dealer may charge a service fee. The other method is to download and print a Lost Combination form from the Master Lock website. After the form is completed, you must take it along with the lock to a notary public and have the form notarized.
You may include up to 6 combinations on one notarized form. Mail the form to: Master Lock Warehouse, 1600 W. La Quinta Rd, Suite/WHSE #1, Nogales, AZ 85621. Your combination will be mailed to you in 4-6 weeks. Please note that if you are an inmate of a correctional facility, the request must be on prison letterhead.
