All Mayo Clinic patient records are kept confidential and secure and are retained as an electronic record, explains Mayo Clinic. If a patient wishes to have his medical records released using an online request, he must first create an online account through the "Patient Online Services" website page at Mayoclinic.org. This is a free and convenient way to access personal medical records. A patient can choose the “Log into Patient Account” option and request a new account. The account creation process is a one-time sign up procedure. By creating this account, a patient has 24 hour-a-day access to a selection of his medical records and personal itinerary.

A patient may also request his medical records to be sent by mail. However, there may be a fee for this service. The authorization form is mailed or faxed to the Health Information Management Services department of the appropriate Mayo Clinic location, states Mayo Clinic.
