California LifeLine Program participants can get new personal identification numbers by mail. An existing customer receives a renewal form in a pink envelope prior to her anniversary date, and this envelope contains a new personal identification number.
To continue their discounts, customers have to complete, sign and return their forms to the California LifeLine Administrator by the deadline. They can also renew online.
Customers who not send the completed form back or renew online by the due date lose their benefits, and customers with home phone service also start paying the regular rates. Consumers who have lost their personal identification numbers or would like to receive new forms can contact the California LifeLine Administrator for assistance.
