Diabetics can download a blood glucose log from the Canadian Diabetic Association's website. The log has several sections for recording blood glucose levels, medications and vitamins.
Diabetics can record their blood glucose goals for before a meal and two hours after a meal at the top of the chart. The chart allows them to record the date and their blood glucose levels before and after each meal, in the evening and at night. They can also note any comments.
The medications section allows diabetics to record the medication name, prescribing doctor, dosage, time of day taken and date started. The vitamins section allows diabetics to record the vitamin name, dosage, how often taken and what they take it for.
