Patients of the University of California San Francisco Medical Center can find their online medical records from the USCFhealth.org home page. Hover over the For Patients button on the main menu, and click on MyChart under Getting Care to access your available medical records.

Sign in, or register to use MyChart by choosing Activate with Access Code or Request an Access Code. These codes are typically given by the health provider at the close of an in-office visit, and are included in the After Visit Summary, or AVS. The access code is good for 60 days.

As of 2015, you may request your code by calling UCSF MyChart Customer Service at 415-514-6000, or by sending an email request to UCSFMyChart@ucsfmedctr.org. Your name, address, date of birth, the last four digits of the Social Security number, and a phone number must appear in the email.
