To claim property listed in the search results, check the box next to the name, and click on the Request Claim Form link below or above the search results. Next, complete the State Claim Inquiry with your name, current address, email address and telephone number, along with other required information. Click on Generate Claim Form To Print, and have the form notarized prior to submission to the Pennsylvania Treasury Department. Send the claim form to the address listed on the form. The state does not accept claim forms online or by email, as of 2015.

When you print your claim form, the database generates a claim identification number. To check the status of your claim, return to the Unclaimed Property Web page, and enter the claim number where indicated on the page.
