The application entails an initial two-part process, which includes the Shelter Plus Care application and the Homeless Management Information System application.

The Missouri DMH only accepts Shelter Plus Care applications by fax. The fax number is on the application in addition to a phone number to call if you need help filling out the application. The Massachusetts Department of Housing and Community Development oversees the Shelter Plus Care program, or the McKinney Shelter Plus Care program.
