New York state disability forms are available for download on the New York State Insurance Fund website. If your disability occurred as a result of a work injury, disability forms may also be obtained from your employer. All employees or recent employees of a New York state employer who have worked at least four consecutive weeks are eligible to receive New York state disability benefits.
Navigate to the New York State Insurance Fund forms website and scroll down to the Disability Benefits Claim Forms section for non-job-related injuries or illnesses. Download and complete the DB-300 form entitled Notice and Proof of Claim for Disability Benefits by Unemployed Claimant. After this form is completed, it must be filed with the New York Disability Benefits Bureau, if you are disabled after being unemployed for four weeks. This form must be completed and filed within 30 days of your disability.
Download and complete the DB-450 form called Notice and Proof of Claim for Disability Benefits. After you complete this form, it must be filed with your employer’s insurance company, if you became disabled while employed, or within four weeks of being unemployed. Like the DB-300, this form must be completed and filed within 30 days of becoming disabled. The DB-450 form is also available in Spanish.
