Click on the Form I-90 link at the top of the I-90, Application to Replace Permanent Resident Card Web page, instructs the USCIS. Print the form once it loads. To access the form's instructions, click on the Instructions for Form I-90 link.
The purpose of Form I-90 is to replace a previously issued card that was lost, stolen, destroyed, mutilated or never received, indicates the USCIS. Also, those with a current card containing erroneous information due to an administrative error, name change or other legal change can complete Form I-90. Cards that are soon to expire or have expired are replaced by completing and submitting Form I-90. Additionally, cardholders who reach the age of 14 are required to submit Form I-90.
