The military form DD-214 is a report of separation between a member of the military and the military branch, according to the National Archives at St. Louis. A military person receives a DD-2141 during active duty or after 90 days of active duty training.
The DD-214 contains all of the information to verify a soldier's military service for retirement, benefits, employment and membership in veterans' organizations. The information on a DD-214 includes the date and place of the start of active duty, home address at the start of active duty, date and place released from active duty, and the home address after active duty. The form also includes last assignment and rank, military specialty, military education, total creditable service, foreign service credited, and separation information. The form may also detail the soldier's decorations, medals, badges, citations and awards received during active duty.
