To obtain a certified marriage statement, download and print an application form from Missouri state's website. On the toolbar at the top of the main page, click Home and Family. On the bottom of the page on the left, click Get a Copy of a Vital Record. Under Step 1, click Application for Copy of Statement Relating to Marriage and Divorce. To complete the application, you need the full name of the groom, the full maiden name of the bride, the date of the marriage, and the city and county where the marriage license was issued. Take or mail the completed form to the Bureau of Vital Records in Jefferson City.
Show valid identification along with the application. Vital records are available only to the individuals involved or their legal representatives and not to the general public.
If you make the request by mail, send a check or money order from the United States Postal Service or a U.S. bank payable to the Missouri Department of Health and Senior Services.
