What Information Should a Sworn Affidavit Include?

A sworn affidavit should include the title of the court proceeding, the name of the parties and their relationships with the parties in the proceedings, explains The Law Dictionary. A sentence affirming the information, a reference to supporting documents and the oath sentence is also included.

The facts of the case should be presented in separate, numbered paragraphs, notes The Law Dictionary. If supporting documentation is presented within the affidavit, these documents are noted as Exhibits. A statement that the information presented is sworn to by the affiant should be included, along with the fact that the sworn statement is a complete representation of the facts. The affidavit should have a signature block for the affiant to affix his signature.
