The applicant also needs to provide a copy of his Social Security statement or record of his earnings. If the applicant was in the military, he also needs to provide the beginning and ending dates of active service before 1968.
If the applicant became unable to work due to injury or illness within the 14 months prior to his application, the applicant needs to provide that information.
