For SSDI, the Application for Disability Insurance Benefits must be completed. Complete the Application for Supplemental Security Income for SSI. For both types of disability, forms to be completed are the Adult Disability Report, the Work History Report, and the Authorization to Disclose Information to the Social Security Administration.
An applicant should ensure that he is using the current versions of the forms. Each form has a revision date on the bottom left-hand corner, so if the revision date is not within the last couple of years, the applicant should contact his local Social Security office to determine if the form is the most current. The applicant should fill out the forms in blue or black ink. An applicant can file online for Social Security Disability Insurance benefits, but not SSI benefits. Some Social Security offices may require the applicant to go directly into an office so a representative can record the answers on the application.
If an applicant decides to file for Social Security Disability Insurance benefits online, he can print the form out and mail it in, or he can sign the form electronically. The applicant must complete the Adult Disability Report form, rather than the applicant's doctor.
