If answer space is insufficient, applicants should attach separate continuation sheets.
Most of the questions on Form I-765 are self-explanatory, according to Nolo. Question 10 asks for an Alien Registration Number, if the applicant has one, or an I-94 number, which is on the record of the applicant's arrival in the United States. In Question 11, applicants write the USCIS office that approved their first work permit and the date the office received their application. Applicants should take particular care in filling out Question 16, their eligibility category, as if this information is missing or incorrect, USCIS denies their application.
Along with Form I-765, people applying for renewal of their work permits must send a Form I-765 Worksheet and the appropriate fee, explains USCIS.
