To fill out a SSI Disability application form, visit the Social Security Administration website and navigate to the appropriate page for application information. Review the Adult Disability Checklist, click Apply for Disability, provide all the requested information in the areas provided, and then click Submit.
During the online application process, you may be asked to provide documentation to prove your eligibility for the program. This documentation can include your birth certificate, proof of citizenship and documented proof of your medical condition that constitutes a disability. W-2's, paycheck stubs and award letters may also be requested as proof of income. Photocopies of most documents are accepted but an original birth certificate must be submitted. All supporting documents can be submitted by mail or in person. When mailing documents, include a separate piece of paper with your Social Security number written legibly on it.
You can save an online application for Social Security disability benefits at any point during the application process. You can come back to the application at a different time and begin right where you left off by clicking the Return to a Saved Application button. Once submitted, check the status of the application by clicking the Check Application Status button. Once your application is reviewed, you are informed of the official decision via postal mail.
