Provide information about your children under the age of 18, including each child's name, social insurance number, date of birth and gender, as well as if each child is adopted or not. Applicants must also furnish information regarding any children they have who are in another person's custody, states Service Canada. If applicants have children between ages 18 and 25 attending an educational institution, they must provide each child's name, address and date of birth. Applicants must specify if they have applied for or received benefits from the Canada or Quebec Pension Plans on behalf of any of their children.
