The Application for Disability Compensation and Related Compensation Benefits form is a general application for all forms of disability benefits available through the VA. Information needed to complete the form includes the applicant's personal information, contact information, a list of disabilities for which benefits are claimed, service details and direct deposit data. Attached to the form is a detailed list of required evidence that must accompany the form, including private or military medical treatment records, additional forms for special circumstances and reports of VA medical examinations. A veteran opting to participate in the Fully Developed Claim Program must submit all required documentation and information on how to find evidence the veteran does not possess.
The Examination for Housebound Status or Permanent Need for Regular Aid and Attendance is a form for an attending physician to complete, giving details on the veteran's housebound status and extent of mental or physical impairment. It includes questions on the veteran's ability to feed himself, dress and undress, use toilet facilities, and remain clean and presentable. Although veterans can submit these forms by mail, the VA recommends an online application through an eBenefits account as the fastest and easiest way to file for benefits.Learn more about Military
