Form DD-1172-2 is an application for the Common Access Card. The DD Form 1172, "Application for Uniformed Services Identification Card DEERS Enrollment" was decommissioned in 2012. DD-1172-2 replaces it and is used for service members and government contractors to receive an ID.
Applicants must give their forms to a sponsor, who is a Department of Defense official or employee, for approval. Sponsors initiate background checks of applicants, including a Federal Bureau of Investigation fingerprint check, and a National Agency Check with Written Inquiries.
The Common Access Card is a smart identification card used by active duty uniformed service personnel, Department of Defence civilian employees and contractor personnel. It is also a smart key that gives physical access to buildings and controlled spaces, and it gives access to Department of Defence computer networks and systems.
