An application, available at a local health department or on the Missouri Department of Health and Senior Services website, must be filled out and presented or mailed to the health department. A check or money order is to be sent in with the application.

To get a birth record without an application, the requester sends in a written and notarized letter with the full name at birth, the date and place of birth and the names of both parents, including the mother's maiden name. The relationship to the person in the birth record and the reason for wanting a copy of the record is to be included, as well.
