To order through mail, Idaho Residents must request and complete the Idaho Vital Statistics Certificate Request Form. This form can be procured at any government office throughout the state. Once completed, mail the form along with a photocopy of a picture identification card and the $13.00 processing fee to the Idaho Bureau of Vital Records and Health Statistics at PO Box 83720 Boise, ID 83720-0036. Standard processing time is five to seven business days once the application is received and verified. For an additional $5.00 fee, the application can be rushed to arrive at the requestor’s residence in two to three business days.
