When you apply, you need to have information on your current housing expenses, including rent or mortgage, utilities, property taxes and insurance. You also need to report any income you have. If you pay for any child care expenses or child support, you are required to report those expenses as well. Finally, if you are asking for assistance in paying medical expenses on this form, you need to report and verify your medical expenses as well.

After applying, it may be determined that you are eligible for an expedited SNAP interview. You may then be asked to come into your local DHS office. The application asks you for information on when you can be available for this interview. However, if you cannot come in for an interview, you must provide on the application a reason as well as your phone number so that an interview can be conducted over the telephone. If, after this process, you are eligible for SNAP and/or cash benefits, a Link card will then be issued to you.
