In order to submit your request, you must first determine in which state the physician practices. This information is used to contact the appropriate state medical board. You also need to know the physician's full name, address and specialty, if applicable.

The Federation of State Medical Boards provides a directory in which you can find the contact information for your state's medical board. You can also fill out an electronic request form directly on the Federation of State Medical Boards website or mail a paper copy. You must also pay a fee to submit this request form. Submit the form online or mail a copy to the address displayed on the form.

Upon receipt of the physician profile report, can learn pertinent information about the physician in question, including his or her DEA number, if applicable. The report lists disciplinary action or sanctions taken against this physician, his or her medical license history, degrees obtained, medical schools attended and locations of practice.
