For question number 12, provide the most recent entry point into the United States, and for "manner of entry" provide the type of visa used to enter the country. For question number 16, specify an eligibility category, such as pending adjustment of status or Temporary Protected Status. Use the attached instructions to determine this category. Depending on the category specified in question 16, applicants may have to supply documentation that proves their status.
