Applying for a Social Security card requires submitting an application and proof of identity either in person or by mail to the nearest Social Security office. Although the application is located on the Social Security website, and it can be downloaded and printed, the application cannot be completed online.
If a birth certificate exists, it must be submitted. If one does not exist, a passport or hospital record may be accepted. Proof of identity must also be shown. Preferred documentation includes a U.S. driver's license, a U.S. passport or a state-issued identification card. Two separate documents must be provided. If the person is applying for the first time for a Social Security number and is 12 or older, he must appear in person for an interview.
