Applying for a new or replacement Social Security card requires printing an application and taking or mailing it to the local Social Security Administration along with the proper documentation proving citizenship and identity. New and replacement cards are free.
As of June, 2015, it is not possible to apply for a new or replacement Social Security card online. Applications must be mailed or taken directly to a Social Security Administration office.

Complete each section of the application as directed.
The Social Security Administration accepts a number of different documents to prove identity and citizenship. A birth certificate, passport, driver's license issued in the United States or state-issued identification card are all acceptable forms of identification. Bring or send documents that verify citizenship and identity. Two separate forms of identification are necessary to complete the process.
Using the Social Security website, type in a ZIP code and click Locate. A list of offices appears on the next screen, along with office hours, mailing address and telephone numbers. Use this information to apply for a card.
