To apply for a United States passport, citizens must have one document that provides evidence of U.S. citizenship and one form of photo identification. Other countries and circumstances may require different documentation.
Documents that provide evidence of U.S. citizenship include a certified birth certificate, previous passport, consular report of birth abroad or certificate of naturalization/citizenship. Valid photo IDs include driver's licenses, previous passports, valid government or military IDs, and certificates of naturalization. Those who do not have these documents, may be able to use a delayed birth certificate, hospital records, religious records or early school records as secondary evidence of citizenship.
