Passport photocopies submitted with the application must include the first six pages for five-year passports and the first 11 pages for older passports. Applicants must also complete form 103 to report a change in residence or obtain permission for multiple exits or form 104 to obtain permission to exit once a year. Husbands must also complete form 111 for accompanying spouses and children. Applicants must provide originals and photocopies of birth certificates for themselves and all accompanying individuals. Birth certificate photocopies must include all pages, and birth certificates for individuals over 15 years of age must contain an ID photo. Applicants must also provide two 3-centimeter by 4-centimeter passport photos, taken within the past six months, for themselves and all accompanying individuals.

Passports may only be renewed upon expiration of the previous passport, if there are no more blank pages in the passport, or if the applicant has proof of registration under a new name, surname or date of birth.
