A DS-82 form is used to renew a U.S. passport by mail or in person. Citizens who have previously had a passport that was issued after the citizen's 16th birthday and who live in the United States or Canada may use form DS-82. Applicants are required to have an undamaged passport to be sent in with the form.
Applicants requesting a name change may not be eligible to use form DS-82 unless the name change can be proved by sending in a certified copy of the marriage license or government-issued paperwork. Form DS-82 can be filled out online, printed, and then mailed for processing or delivered in person at a passport office or authorized agency. Applicants should have the passport, emergency contact information and a printer when filling the application out online.
Forms also can be obtained in person at a passport office or agency. U.S. citizens living abroad are not eligible to mail in passport renewal applications and must apply in person at the nearest U.S. consulate or embassy. Passports submitted for expedited renewal are subject to additional processing fees. As of September 2015, processing times are four-to-six weeks for standard renewals, three weeks for expedited service, and eight business days for expedited agency-processed renewal applications. Agency-processed renewal applications are granted based on need.
