Both types of permission slips are designed to remove liability from the school in case of accident or injury. Permission slips typically include the name of parent/guardian and child; the date of the field trip; the location of the field trip; the purpose of the trip; the cost; the means of transportation; the departure and arrival time; permission to treat the child in a medical emergency; and contact information should an emergency arise. Notarized permission slips are often used when the field trip takes place in another city, state or country and when the field trip includes an overnight or longer stay.
