Before you proceed, you must be 18 years or older. If you are under the age of 18 and a representative payee, you have to complete the report form you receive in the mail and return it to the address specified on the form. In order to access your beneficiary’s records, you are required to have the report you received in the mail.
In most cases, it takes roughly 15 minutes to answer the questions after gathering the facts and reading the instructions. It is a requirement that you fill this report in only one sitting. If you do not complete the report in one sitting, you cannot save the report. The good news is that you can log in again and start writing the report from the beginning.
After you are through with writing the report, you receive a confirmation number for your reference. This confirmation number is saved for up to 30 days for your review.
