Once on the IRS website and the Forms & Pubs link is selected, the next page has a list of a few different tax publications but not the W-9 form. In order find the W-9 form, select the link in the middle of the page labeled: Find All Curret Forms & Pubs. This link accesses a page containing other tax documents and a search text box labeled which is near the top of the page. In the text box type in "W-9" and select the link labeled Find.
After the search is performed, the W-9 form appears at the very top of the list of forms as a hyperlink. Selecting the hyperlink brings up the W-9 form as a PDF document which has sections for filing status, name, address, social security information, and a signature and date section at the bottom of the first page.
