A VEBA claim form is a reimbursement request form filed by government employees for out-of-pocket health expenses. Only participants and their dependents in the Voluntary Employees’ Beneficiary Association are eligible for the claim, according to the HRA VEBA Trust. A VEBA beneficiary files claims if he is claims-eligible.
Those filing the claim forms can do so at their convenience, but must be VEBA enrollees. Eligible expenses include co-insurance, dental, eye care, prescriptions and co-pays, according to HRA VEBA Trust. Medicare plans and long-term insurance premiums are also eligible.
Beneficiaries fill in the claim forms online before submitting them. A proof of expense should also accompany the claim form, to serve as supporting documentation for a claim, says SelectAccount.
