Standard enrollment for Kaiser Permanente Medicare health plans can be conducted through a step-by-step online form available on Kaiser's website. Enrollees may also speak with a licensed sales specialist by calling a toll-free number between the hours of 8 a.m. and 8 p.m. seven days a week. Local sales specialists are also available by appointment, and enrollees can schedule by contacting Kaiser. Toll-free numbers are available on Kaiser's website. Medicare beneficiaries may also enroll through a government website. Medicare health plans are available through the CMS Online Enrollment Center on Medicare.gov.
Enrollment forms may be submitted through the mail. Forms, requirements and mailing addresses vary depending on the state of residence. State instructions, forms and addresses are available on Kaiser's website. Many state forms, such as California and Georgia, offer the form in Spanish as well as English. Other states, such as Maryland, Virginia and Northern and Southern Colorado, have requirements for choosing a primary care physician at the time of enrollment.
