The performance evaluation form offers spaces for the employee's name, position and department and the date of the appraisal. There are typically categories to rate, including job knowledge, quality, productivity, attendance and relationships. The supervisor can rate the employee on a scale from "not applicable" to "excellent."
A vendor performance evaluation form lets companies see how their vendors perform. The form includes a space for the vendor's name and address, items supplied, date and person evaluating. There are more than 10 categories in which to rate the vendor, including service, on-time delivery and handling of complaints.
