The Occupational Safety & Health Administration publishes a printable OSHA 300, Log of Work-Related Injuries and Illnesses, form on its website, OSHA.gov. The form is available as part of a 12-page booklet that includes Forms 300, 300A and 301, as of 2015.
To find the booklet, visitors click on Publications from the home page, scroll down to the link for Recordkeeping: OSHA 300, 300A, and 301 Forms, and click on PDF to open the booklet. The form includes an overview of recording work-related injuries and illnesses, and information on calculating injury and illness incidence rates. It also includes information on filling out the Log of Work-Related Injuries and Illnesses and preparing Form 300A, which is the Summary of Work-Related Injuries and Illnesses, and Form 301, which is the Injury and Illness Incident Report.
