The PPO plan is referred to as The Empire Plan and is administered by NYSHIP under United Healthcare. As of April 2015, there are over 100,000 health care providers that are a part of the network. Additionally, consumers can receive physical therapy or chiropractic services from one of over 5,000 providers.

Employees interested in the HMO option have 10 different providers to choose from. These providers include companies such as Aetna, Blue Cross Blue Shield of WNY, Capital District Physicians' Health Plan, Empire Blue Cross Blue Shield and Blue Choice. GHI HMO, an Emblem Health Company, along with HIP Health Plan of New York, HMOBlue, Independent Health and MVP Health Care are the remaining options that employees have to choose from. The choices available to employees may vary based on their location.
